Manuscript Checklist

_____ Figures are to be sent electronically in digital format EPS or TIFF files, Resolution 1200 for black and white images, 600 dpi grayscale mode for image blots, and 300 dpi CMYK mode for color images.  A figure should be no larger than 16 cm in height at the appropriate resolution.  Figure legends should be typed double-spaced on a separate sheet and should be numbered consecutively in the text.

_____ Regardless of submission format (e.g., electronic or hard copy), one disk containing the manuscript in Microsoft Word or WordPerfect should be mailed to the Editor.

_____The manuscript may be submitted to the Associate Editor, Sherry Luedtke, at editor@jppt.org

 TITLE PAGE

_____  Concise, informative title that contains the key points of the work.

AUTHOR IDENTIFICATION (following title on the same page as title page)

_____  First name, middle initial, last name and highest academic degree of each author.

_____  Institutional affiliation. (use superscript number that corresponds to each author)

RUNNING TITLE (following author identification on the same page as title page)

_____  A brief title that consists of few than 50 characters.

KEYWORDS (following Running Title on the same page as title page)

_____  No more than 5 words in alphabetical order and according to Medical Subject Indexing.

CORRESPONDING AUTHOR INFORMATION (following keywords on the same page as title page)

_____  Includes mailing, phone and fax numbers and e-mail address

ABSTRACT (Required for review articles, clinical investigations, and case reports)

_____  Follows the author and title page.

_____  Less than 300 words.

_____  Clinical Investigation manuscripts should concisely state objectives, methods, results, and conclusions.

TEXT

_____ Descriptive uppercase bold headings are used to identify major sections of the paper, subheadings may also be used.

_____Original research manuscripts should include a brief review of the work in the field, intent of the research (hypothesis), methods (including statistical approaches), results, discussions, and conclusions.

_____ Reports involving experiments in humans must indicate whether the procedures followed were in accord with the ethical standards of the institution’s committee on Human Experimentation or in accord with the Helsinki Declaration of 1975.

_____ Drugs and chemicals should be identified by their generic name.  Trade names should be listed once, unless a specific trade name preparation is being discussed.

_____ For commercial products important to the paper, denotes, in parentheses: trade name or model number; the manufacturer’s name, city, state; and the lot of number if applicable.

_____ Measurements are reported in the units in which they were made.  Volume, distance, and weight measurements should be expressed in the metric system.

_____ Names of authors and institutions are not to be mentioned, except in procedural reviews where institution identification is essential to understanding the program.

_____ For case reports: patient’s age, gender, race, weight, pertinent medical history, and baseline laboratory values are included.  Patients should be referred to using a number (no initials).

_____Identifies in the methods section all statistical tests used and the a priori level of significance; unusual or complex statistical methods are explained briefly or referenced.

_____ If more than one statistical test is used, the data evaluated by each test are clearly identified in the methods section.

_____ Mean values for study results are accompanied by some measure of variability or precision.

_____ References, figures, tables, and appendixes are cited in the text in numerical order.

ACKNOWLEDGEMENT

_____ Indicate if an abstract or any portion of the manuscript has been presented at a meeting.  Include the name of the organization, place, and date on which it was presented.

_____ List all persons who have made substantial contributions to the work reported in the manuscript, but who are not authors.

DISCLOSURE (CONFLICTS OF INTEREST)

_____ Authors should list any conflicts of interest pertinent to the content of the manuscript and should acknowledge all source(s) or support (e.g., grants, equipment, drugs, or all of these). Dowload conflict of interest form.

REFERENCES

_____ A word processing program does not generate the reference numbers, such as automatic footnotes or endnotes.

_____ Cited consecutively in the order in which they appear in the manuscript.

_____ Identified in text, tables, and legends, by superscript Arabic numbers.

_____ References cited in tables and figure legends are also numbered consecutively as they appear in the text.

_____ Reference style is consistent with the Uniform Requirements for Manuscripts submitted to Biomedical Journals (www.icmje.org/index.html).  See www.jppt.org for document and examples of reference style.

_____ References to a journal include:

1)       Lists of authors and/or editors up to 3, if more than 3 list et al.
2)       Complete title as it appears on the publication
3)       Journal name as abbreviated in Index Medicus
4)       Year
5)       Volume number
6)       Inclusive page number – in that order

_____ References to books include:

1)       Lists of authors and/or editors up to 3, if more than 3 list et al
2)       Chapter title – if any
3)       Editor – if any
4)       Title of book

_____ Web References should include the URL and the date on which information was obtained.  Please keep a printed copy of any reference to Web-only information.

_____ Authors have verified the accuracy of cited references and have confirmed that each reference appears in the text of the manuscript.

_____ Authors have obtained written permission from any individual named as a source for information in a personal communication via: conversation, letter, e-mail, or telephone call.

TABLES

_____Typed double-spaced.

_____References cited in tables are numbered consecutively as they appear in the text.

_____Tables are electronically submitted in Microsoft Excel or Microsoft Word and are included in a separate file.

_____Title and footnotes for each table are included on a separate page.

_____Use superscript letters for footnote designations.

_____If data from another published or unpublished source is used, permission is obtained from the source (proof submitted with paper) and the source is acknowledged.

FIGURES
